2. Improper Angulation of Grafts

The extent of the damage and remaining donor reserves will play an important role in what is possible and how good the result can be. Don’t make the same mistake twice. If your previous surgeon significantly missed the mark the first time due to unsound practices, they are unlikely qualified to identify, plan and execute a successful repair.
Limited Options
Options for revision may be limited, emphasizing the importance of preventing errors in the initial procedure. Scar tissue, depleted donor areas, and altered scalp characteristics pose challenges that may restrict the feasibility of certain corrective approaches. Therefore, meticulous planning and execution during the primary transplant are paramount to minimize the need for revisions.

This patient had older technology with large grafts that looked unnatural. While his surgeon was obviously not the right choice, thankfully the hairline was placed high enough that the repair was not too difficult. Instead of needing to remove the old work, we could simply reconstruct a new natural hairline that was lower and completely hide the previously transplanted grafts. This patient was lucky that we could reverse this unnatural looking mistake and achieve this result in one session. Others may not be as lucky and multiple procedures my be required.

What Are Hair Plugs?

Older hair transplant procedures from as early as 1952 up until the 1980’s were commonly referred to as hair plugs. Large round punches were used to harvest groupings of hair that would commonly consist of up to 30 hairs per unit or graft. This now outdated approach to hair transplant surgery resulted in an unnatural outcome resembling the appearance of doll’s hair due to large graft size and spacing between plugs. The donor area was also negatively impacted as the harvest of these large grafts left a considerable amount of potentially noticeable scarring and damage that would also limit further harvests. With advancements in technology and surgical techniques, such as follicular unit transplantation (FUT) and follicular unit extraction (FUE), the era of hair plugs is thankfully behind us. These modern methods involve the transplantation of individual follicular units, resulting in more natural-looking hairlines and improved aesthetic outcomes, while minimizing scarring and at the same time enabling a faster

Hair Plugs vs Hair Transplant

  • Natural Result: Both FUT and FUE provide much more natural-looking results compared to the outdated hair plug method of extraction.
  • Reduced Scarring: Modern techniques in the right hands leave minimal scarring, making it difficult to detect that a hair transplant was performed.
  • Improved Aesthetic Outcomes: The smaller grafts which therefore require smaller and more precise incisions enable better graft placement and integration with the existing hair, creating a more cosmetically pleasing outcome.
  • Enhanced Recovery: Patients experience quicker recovery times and less post-operative discomfort and side effects with these newer more modern methods.

Hair Plugs Repair

When adopting a plan for a hair plug repair, the surgeon carefully evaluates the patient’s scalp to determine the best course of action. This includes determining whether the location of the plugs is appropriate and if the angle and direction is acceptable. If they are, the repair can be easier if the surgeon can eliminate the negative appearance of the plugs by transplanting the necessary number of grafts in front of and around the old work to completely camouflage.

In more difficult scenarios, removing the outdated plugs is necessary for the best outcome. Removed plugs can be broken down into the correct units and repurposed elsewhere on the scalp. After a period of healing, the newly created landscape, usually the hairline and frontal zone, can then be reconstructed using the modern techniques mentioned above to achieve the desired natural result. As long as the previous work has not severely compromised the donor hair and there remains a sufficient number of grafts available in reserve, outcomes can be very good.

In those cases where donor hair is very limited and there is not enough in reserve to adequately repair the previous work, removal without reconstruction is also an option. No hair or a bald appearance will be more natural than the doll hair/plug look. Ultimately, hair plug repair offers a transformative solution for individuals dissatisfied with the unnatural results of previous hair transplant procedures. By combining surgical expertise with modern techniques, patients can often reclaim a natural-looking, full head of hair and regain confidence in their appearance.

This patient learned the hard way that choosing a surgeon based exclusively on price is likely going to be a poor decision. Opting for a very low-cost procedure in a foreign country did not achieve the expected results, leaving him in worse condition than before. After experiencing this very disappointing outcome, he conducted further research and discovered that Hasson and Wong were far superior to his previous doctor, and he hoped we could correct his earlier mistake. Fortunately, he still had enough donor hair for a significant transformation. After removing all of the old hair plugs deemed necessary, 3,170 grafts were used to reconstruct the new hairline and frontal zone. The extra steps taken by Dr. Hasson and his team, including an additional removal session, ultimately paid off. The results speak for themselves, and although the process was costly and time-consuming, it was a life-changing transformation for this patient.

This patient also had large, unsightly grafts from a previous procedure. Despite issues such as poor angle and direction, the previous surgeon did not place the hairline too low or advance the temples. Therefore, removing the previously transplanted grafts was unnecessary. Dr. Hasson could design and reconstruct a lower hairline to effectively camouflage all the old grafts.

Correct Angle And Direction

In many repair cases, previously transplanted grafts are placed at incorrect angles and directions. If these misalignments are not too extreme, they can be overcome with a lower hairline and sufficient density. By placing enough new grafts in front of and around the old ones, the new hair can influence the appearance of the old grafts, allowing them to blend in seamlessly with the new grafts.

Final Results

The top priority was to restore a natural appearance to the front, using any remaining donor reserves necessary to achieve that goal. Often, in cases like this, donor reserves are compromised, limiting what can be done. However, this patient was fortunate to have a good remaining donor supply. As a result, Dr. Hasson and his team could not only restore the hairline, frontal zone, and mid-scalp, but also completely cover the entire balding areas, including the crown, achieving a remarkable transformation.

 Hair Plugs Cost

In such a case as referenced above where old hair plugs are appropriately placed with correct angle and direction, the cost can be similar to a typical reconstruction on a virgin scalp. However, in cases that require hair plug removal, costs can be greater. In addition to the removal phase or phases, some cases will require not only one but perhaps two sessions to reconstruct.

A few scenarios and how they may affect cost:

  1. No need for old plug removal: The cost will be similar to a hair transplant procedure on a virgin scalp.
  2. Some removal of plugs required and one session to reconstruction after removal: 25 – 50% above the price of scenario #1.
  3. Extensive removal of plugs requiring two removal sessions: closer to 50% above the price of scenario #1.
  4. Extensive removal of plugs requiring two removal sessions and two sessions or more required after removal: 100% or more above the price of scenario #1.

 Hair Plugs Scars

Scars from hair plug procedures are more noticeable when hair is cut to shorter lengths on the sides and back. In addition to an unnatural hairline, the resulting donor area scarring can also be an issue. In the best possible outcome, both the recipient area and the donor area can be improved. In a typical case, as much of the scarring from the hair plug harvest will be excised reducing the total area of scar tissue. The extent of the scarring among other factors can influence the best approach but because a hair plug harvest usually leaves a large area of scarring creating areas of lower density as well, FUT can be the preferred method of extraction provided the scalp has sufficient laxity. By removing as much of the scarring as possible, its impact on the appearance is reduced. At the same time, grafts harvested within the strip are used to reconstruct the new hairline. FUE can also be considered to add hair into the pencil eraser sized scars. However, in these cases the patient will need to have enough remaining donor hair. We’ll need enough grafts to completely reconstruct the hairline and any other necessary areas in addition to having enough to fill in the donor area scars.

Hair Plugs Summary

With the arrival of more advanced techniques like FUT and FUE, the transition from hair plugs marked a significant improvement in hair transplant surgery results. These modern methods are not only superior to older methods of the past but also reduce the negative impact on the donor area. Thankfully, hair plug procedures are mostly a thing of the past, but unfavorable techniques and procedures can still be found today. While it is far less likely you’ll receive a procedure that achieves a result more typically found decades ago, choosing a clinic or surgeon with extensive experience in modern techniques to ensure the best possible outcome is still important today. A “repair” patient is the last thing you’ll want to be considered because of a bad hair transplant decision.

How Hair Transplants Can Transform Appearance

We all want to look in the mirror and like what we see. For those of us who experience hair loss, it starts slowly and gradually becomes more of a distraction and presents issues when we try and style our hair to make it look presentable. Each passing month and year offers new challenges and eventually for some of us there is very little if any hair in the front to style.

Over time with continued hair loss the mirror becomes a reminder that we just don’t feel as good about our appearance anymore and certain things in our life may start to suffer as a result of our lowered self-esteem. For some of us there comes a point in time when we’ve had enough and we would like to seek solutions for this problem. In the case of this Vancouver area patient he was fortunate enough to have the answer right in his backyard so he decided pay a visit to Hasson and Wong.

As is the case with many patients, a full head of hair was the ultimate goal. For some this is possible, for others expectations need to be slightly or substantially tempered. When Dr. Hasson evaluated this stage 6 patient he determined his donor hair was sufficient enough to provide reasonably good coverage to the considerable amount of balding scalp. It was explained that with this large area to cover in mind, this would be a two-stage process.

The first session would rebuild a new hairline, restore the frontal zone and mid scalp working to the front of the crown. This would provide a significant cosmetic benefit by re framing the face and therefore take many years off of his appearance. Dr Hasson and team accomplished this goal using 5474 grafts obtained using the FUT method of extraction. For stage 6’s or 7’s on the Norwood scale who have a considerable amount of area to cover, FUT is usually the preferred method for it’s ability to provide a few more grafts in total over FUE.

A second procedure, if desired, would complete the coverage by restoring the crown. For the second procedure, it’s likely either an FUT or FUE option would be possible. You can see in these photos the hairline design suits the patient well and the predictable transformation is quite impressive. It also goes without saying that the patient was very happy with the result and positive affect it has had on his self-worth.

This is just another good example of what a hair transplant can do for one’s appearance and how they feel about themselves. These are essentially two totally different people. One being the balding aging man unhappy with his appearance before the procedure, the other being a more youthful and confident handsome looking man after. He feels a little different about those trips to the mirror these days.

He Went To Turkey To Save Money On A Hair Transplant

Hear What He Has To Say About His Decision

Stephen Mason was just 22 years old when he first began to lose his hair. By the age of 28 his hairline had receded noticeably, and by age 30 he was starting to experience thinning throughout his frontal region, mid-scalp and crown. As is the case with many young men in his situation, Stephen’s hair loss dealt a significant blow to his confidence, prompting him to look for ways to address – and hopefully even reverse – the situation.

It wasn’t long before the concept of hair restoration surgery caught his attention. Looking to gain more information on the subject, Stephen approached Hasson & Wong for an initial consultation in December of 2016. During the consultation, Dr. Hasson confirmed what Stephen already suspected: that he was indeed experiencing male pattern hair loss, and that the sooner he sought treatment for it the better. He recommended a 3500-graft FUE surgery to the frontal zone of his scalp, as well as Proscar to prevent further hair loss in his mid-scalp and crown.

Unfortunately, Stephen found the cost of the hair transplant at Hasson & Wong to be somewhat prohibitive and decided instead to pursue his options abroad. He travelled to Istanbul, Turkey in March of 2017, where he underwent a 3500-graft FUE procedure at what he considered a much more reasonable price. Having failed to conduct proper research before selecting a clinic, however, he fell victim to the idea that all hair transplant surgeries are created equal. While there are a select few capable surgeons in Turkey, Stephen had unluckily found himself at a less reputable establishment – with the results to prove it.

Stephen Shares His Results At His 6 Month Post-Surgery Mark

Dissatisfied with the outcome of his procedure in Turkey, Stephen approached Hasson & Wong for advice. Upon evaluation of the work done in Istanbul, Dr. Hasson noted that the hairline and tissue exhibited ridging, cobblestoning, misdirected hairs, multi-hair grafts and low overall density. In addition, his temporal angles had been blunted – resulting in the unnaturally-shaped and ‘pluggy’ hairline characteristic of a poorly performed procedure. A well-executed modern hair transplant should never present any of these characteristics, as they are unnatural and thus prone to drawing the wrong type of attention. Thankfully, Dr. Hasson agreed to perform a second procedure on Stephen in order to repair the damage incurred in Turkey. Shortly after his consultation, he returned to the clinic to have 95 misdirected grafts in the temporal angles removed – a sacrifice necessary for the creation of the masculine, natural-looking hairline he has today. Dr. Hasson and his team then went on to rebuild his hairline and frontal zone by means of a 2000-graft FUE procedure. Of these grafts, 343 were single-hair grafts, 1329 were doubles and 328 were 3-4 – hair grafts. A year later, he now boasts a full, dense and entirely natural-looking head of hair.

Summary

Sadly, Stephen’s story is not an uncommon one, and it should serve as a warning to all: when it comes to cosmetic procedures of this nature, you truly do get what you pay for. As Stephen himself remarked, ‘if you’re trying to get a hair transplant for the best price, then you shouldn’t get a hair transplant at all’. Fortunately for Stephen, we were able to salvage his result – but this is not always the case. So whether you elect to have surgery at Hasson & Wong or a reputable clinic abroad, it’s best to do your research and get it right the first-time round.

A Brand-New Man

Originally from Abbotsford, this patient in his early 50’s came to Hasson & Wong hoping to address his heavily receded hairline and thinning crown. He started losing his hair at the relatively young age of 22, and his hair loss gradually worsened over time – eventually putting him at stage 5-6 on the Norwood Hair Loss Scale.

Upon examination of his scalp, Dr. Hasson determined that he was a candidate for a large-scale FUT procedure dedicated to restoring maximum density to his frontal zone and crown. On the day of surgery, Dr. Hasson and his team of skilled technicians transplanted a total of 6514 grafts into his frontal zone, mid-scalp and crown during a single day-long session.

At first glance, it’s hard to believe that the two people in these images are in fact the same person. His hair transplant has easily made him look 10 years younger! Needless to say, he left the clinic a completely changed (and very happy) man.

Unrecognizable For All The Right Reasons

Originally from the USA, this patient in his mid 30’s came to Hasson and Wong hoping to restore density to his thinning hairline, frontal zone and crown. He first started losing his hair at the age of 28, and rapidly progressed to a Stage III Vertex (Diffuse Pattern Alopecia) on the Norwood Hair Loss Scale – characterised by recession in the hairline (particularly the temple regions), as well as thinning in the crown. Without surgical intervention, he would likely have progressed to a Stage VI. Upon examination of this patient’s scalp, Dr. Wong determined that he was a candidate for a large-scale FUT surgery (4000-4500 grafts) dedicated to restoring his hairline and frontal zone. Due to the advanced extent of his hair loss, however, he would most likely require a second surgery to restore density to his crown.

During the patient’s first surgery in May 2016, Dr. Wong and his team successfully harvested 4527 grafts via FUT and transplanted them to his hairline and frontal zone. The surgery itself was very successful, and his post-op procedures in the following days went smoothly. His second surgery took place over three years later in November 2019. During this procedure, Dr. Wong and his team harvested 3820 grafts via FUT and transplanted them into the patient’s crown.

The transformation to the patient’s frontal zone is undeniably impressive: he now boasts a full, dense hairline that can be styled in any manner he desires.  Over the course of the next 11 months, we expect equally great things from his crown!

Behind The Blur, There’s A Smile

This Vancouver BC-based patient in his mid-40’s approached Hasson & Wong hoping to restore his hairline and frontal zone to its former glory. During his in-person consultation at the clinic, Dr. Hasson observed that he had exceptional donor hair as well as a tighter-than-average scalp – making him a good candidate for a large-scale (4000+-graft) FUE procedure. These grafts would be dedicated to rebuilding patient’s hairline and frontal zone.

On the day of surgery, Dr. Hasson and his skilled team of technicians harvested 4035 grafts via the FUE extraction method and transplanted them into the desired regions. Only 9.5 months later, this patient would go on to boast one of the most spectacular results the clinic has ever seen. He now has a full, entirely natural-looking head of hair that frames his face in a manner that enhances his masculine features.

This Vancouver-based patient in his early 30’s approached Hasson & Wong seeking a long-term solution to his receding hairline and weak frontal zone. Originally from Vancouver, this patient in his late 20’s came to Hasson & Wong in order to rebuild his receding hairline and thinning frontal zone. He first started losing his hair at the relatively early age of 20, most likely due to a genetic proclivity to male pattern hair loss on his father’s side. Why Crown Hair Restoration Requires A Highly Skilled Surgeon

Men often experience hair loss differently. While some may experience a receding hairline, others may lose hair on the top, back part of the scalp commonly referred to as the crown. Just as one might expect, experienced surgeons must take a different approach when restoring hair in the crown than they do when re-creating a hairline. The Natural Whorl Pattern in the Crown

Unlike the rest of the scalp, hair grows in a circular pattern in the crown, commonly referred to as the whorl or swirl.   Many average doctors can transplant hair. But strategically creating incisions in order for hair to appear both dense and natural is an art form that only a handful of outstanding surgeons possess.   The hair transplant surgeon must create tiny incisions placed closely together at varying angles that mimic the natural swirl.  Simply creating random incisions, even if they are placed closely together might grow hair, but it simply may not look natural, can result in styling restrictions and it may be obvious to the public that something just isn’t right.  Moreover, implanting the actual follicular units into the incisions correctly can be challenging. It is important that the technicians don’t inadvertently damage the grafts or harm the surrounding ones in the implantation process.

Crown Hair Transplant Timeline

No matter where on the scalp the hair is placed, the hair transplant timeline is essentially the same with slight variations. It is said that hair placed in crown might take a little longer to grow, it typically takes a full year and, in some cases, up to 18 months for the hair to fully mature.

After a crown procedure, the recipient area will take about a week or two to appear fairly well healed.  Four to six weeks later, most if not all of the transplanted hair in the crown will shed in most cases. After about 3 to 4 months, tiny hairs lacking full pigment will begin to sprout although they won’t all grow at the same time. Over the next couple of months, these hairs will darken and grow as they mature and begin to look more like normal hair. After about six months, typically the transplanted hair will be approximately 60 to 70% mature.   Finally, after one year, most of the transplanted hair will have fully matured although sometimes it may take up to 18 months as previously mentioned. Some “slow growers “may experience delayed growth.

 

How Much Does a Crown Hair Transplant Cost? 

The cost of a hair transplant to address the crown is essentially the same as a procedure to address other areas such as the hairline/front or mid-scalp. But the cost does depend on a number of factors. For starters, the type of procedure will affect the price. For example, follicular unit extraction also called FUE is typically more expensive than follicular unit strip surgery or FUT. Secondly, clinic location can affect the price.  But even in the same location, one clinic might charge one price while the price will be different at another. In addition to the other factors, the total cost of the procedure often revolves around the number of follicular units being transplanted. That said, cost per graft for FUT typically ranges from $4 to $6 per graft (please confirm pricing with a representative as these prices are subject to change). While cost per graft for FUE can range from $6 to $8 per graft (please confirm pricing with a representative as these prices are subject to change).   For large procedures performed at our clinic where any grafts over 2,000 are priced lower than the first 2,000, the average cost per graft will be reduced. For this reason, one can argue that the more grafts necessary or possible, the greater the value. One must also be aware that not all clinics are not created equal and the quality of a clinic’s result is not necessarily related to their pricing. You may underpay or overpay for the same substandard result.

 

Does the Crown Area Grow Slower Than Other Areas?  

Some hair transplant patients have reported slower growth in the crown than the hairline and mid-scalp areas. But is this actually true or is it just an illusion?  While all hair can grow at different rates, there is no real evidence that transplanting hair in the crown will cause it to grow slower than the rest of the scalp. Because hair takes on a different pattern in the crown, it may appear that hair in the crown is growing slower because the area may be thinner than the rest of the scalp.  This may be due in part to the whorl pattern, as hair fans out from a central location, it may not appear as dense as hair in the front that will have the benefit of a more hair heading in the same direction. But because the donor hair supply is limited and the cosmetic impact is greatest with reconstruction of the hairline and frontal zone versus the crown, surgeons may limit the number of grafts they use in the crown to create an illusion of density. Committing too may grafts to the crown may mean sacrificing some density in the hairline and mid scalp areas which may compromise the quality and naturalness of the result. Patients with above average donor hair and potential future needs that will not exhaust the donor supply can have greater densities safely placed in the crown. The patient referred here would be one of those cases.

What Are Hair Plugs?

Older hair transplant procedures from as early as 1952 up until the 1980’s were commonly referred to as hair plugs. Large round punches were used to harvest groupings of hair that would commonly consist of up to 30 hairs per unit or graft. This now outdated approach to hair transplant surgery resulted in an unnatural outcome resembling the appearance of doll’s hair due to large graft size and spacing between plugs. The donor area was also negatively impacted as the harvest of these large grafts left a considerable amount of potentially noticeable scarring and damage that would also limit further harvests. Often, in cases like this, donor reserves are compromised, limiting what can be done. However, this patient was fortunate to have a good remaining donor supply. Hair Plugs Cost

In such a case as referenced above where old hair plugs are appropriately placed with correct angle and direction, the cost can be similar to a typical reconstruction on a virgin scalp. However, in cases that require hair plug removal, costs can be greater. In addition to the removal phase or phases, some cases will require not only one but perhaps two sessions to reconstruct.

A few scenarios and how they may affect cost:

  1. No need for old plug removal: The cost will be similar to a hair transplant procedure on a virgin scalp.
  2. Some removal of plugs required and one session to reconstruction after removal: 25 – 50% above the price of scenario #1.
  3. Extensive removal of plugs requiring two removal sessions: closer to 50% above the price of scenario #1.
  4. Extensive removal of plugs requiring two removal sessions and two sessions or more required after removal: 100% or more above the price of scenario #1.

Hair Plugs Scars

Scars from hair plug procedures are more noticeable when hair is cut to shorter lengths on the sides and back. In addition to an unnatural hairline, the resulting donor area scarring can also be an issue. In the best possible outcome, both the recipient area and the donor area can be improved. In a typical case, as much of the scarring from the hair plug harvest will be excised reducing the total area of scar tissue. The extent of the scarring among other factors can influence the best approach but because a hair plug harvest usually leaves a large area of scarring creating areas of lower density as well, FUT can be the preferred method of extraction provided the scalp has sufficient laxity. By removing as much of the scarring as possible, its impact on the appearance is reduced. At the same time, grafts harvested within the strip are used to reconstruct the new hairline. FUE can also be considered to add hair into the pencil eraser sized scars. However, in these cases the patient will need to have enough remaining donor hair. We’ll need enough grafts to completely reconstruct the hairline and any other necessary areas in addition to having enough to fill in the donor area scars.

Hair Plugs Summary

With the arrival of more advanced techniques like FUT and FUE, the transition from hair plugs marked a significant improvement in hair transplant surgery results. These modern methods are not only superior to older methods of the past but also reduce the negative impact on the donor area. Thankfully, hair plug procedures are mostly a thing of the past, but unfavorable techniques and procedures can still be found today. While it is far less likely you’ll receive a procedure that achieves a result more typically found decades ago, choosing a clinic or surgeon with extensive experience in modern techniques to ensure the best possible outcome is still important today. What Is Shock Loss?

It is important to distinguish the difference between “temporary” shock loss and “permanent” shock loss. We will first discuss temporary shock loss and further discuss permanent shock loss later in the article.

Shock loss (temporary), also referred to as telogen effluvium, is a common and typically temporary phenomenon that can occur following a hair transplant procedure. It involves the shedding of both transplanted and existing hairs due to the stress the scalp and follicle undergoes during a procedure.

Hair growth occurs in four stages. Each stage plays an important role in the life cycle of hair. The four phases are:

  1. Anagen (growth phase)
  2. Catagen Phase (transition phase)
  3. Telogen Phase (Resting Phase)
  4. Exogen (shedding phase)

Some experts consider the telogen phase the shedding phase as well.

Disruptions in the cycle, such as a hair transplant procedure, can lead to hair loss conditions such as shock loss/telogen effluvium, where many hairs enter the telogen or exogen phase prematurely.

Hair Transplant Shock Loss Timeline

When will you begin to notice the first signs of shock loss? Understanding the timeline of shock loss following a hair transplant can help manage expectations and alleviate concerns. Here’s a breakdown of what to expect:

Early Phase of shock loss (Weeks 2-8)

Weeks 2-4: Noticeable shedding of transplanted hairs starts

The shedding of both transplanted and native hair is often called “shock loss” and is a normal part of the hair transplant process. Hair can start to shed within a few days of the procedure but most of the transplanted and existing hair will remain in the first 2 weeks. Often at the 1-2 week mark, you get a preview of what’s to come and things are looking pretty good. At that 2 week point you may notice the hair starting to thin in mostly the recipient areas. Usually by week 4-5, all the transplanted hair and any of the existing hair that may have been shocked will fall out. In a very small percentage of cases, the hair will not fall and continue to grow from day one of the transplant. However, you should not count on this rare occurrence.

Weeks 4-8: Continued shedding of transplanted hairs.

During this time, most of the hair will have shed due to the trauma of the procedure. While some hair can continue to shed after 5 weeks, most all the hair that will shed will do so by week 5.

Intermediate/final Phase

Months 2-3: Peak period for shock loss. Most all the transplanted hair will have shed and some native hairs might have also shed, leading to a temporary decrease in hair density. Considered the “ugly duckling” phase, this period is when you may look the thinnest.

Months 3-4: Shedding is mostly done and new hair growth from the transplanted follicles begins. These new hairs might be fine and lighter in color initially. Typically, at month 4, you will have more hair and look better than before the procedure or very close to turning the corner. In some cases, there is a notable improvement even at just 4 months post-op. Each individual’s timeline can vary.

Does everyone get shock loss after a hair transplant?

Not everyone experiences shock loss after a hair transplant, but it is quite common. The extent and occurrence of shock loss can vary widely from person to person. Several factors influence whether someone will experience shock loss and how severe it might be:

Skill of the Surgeon:

Experienced surgeons using advanced techniques can minimize the trauma to the scalp, reducing the risk of shock loss. Precise angle and direction control of the recipient site incisions using smaller custom sized blades can reduce the chance of shock loss.

Procedure Type:

Different hair transplant techniques, such as Follicular Unit Extraction (FUE) and Follicular Unit Transplantation (FUT), might have varying impacts on shock loss. The type of method used for extraction should have little to no effect on shock loss in the recipient area. However, shock loss in the donor area can be affected differently by each method. With FUT, there can be some temporary shock loss above and/or below the incision that will grow back. “Permanent” shock loss can also occur if a lesser skilled surgeon transects some of the follicles along the incision line. In this case that area where hair will not grow back just parallel to the scar line can in essence visually increase the width of the scar. With FUE, temporary shock loss can also occur depending on the punch size and other factors including the technology and skill of the extractor. Permanent shock loss can also occur if extraction techniques and technology are poor, the donor area is overharvested or there is a general lack of skill by the operator. At Hasson and Wong, temporary shock loss in the donor area is uncommon, permanent shock loss is averted.

Scalp Sensitivity And Shock Loss:

Some patients will have a more sensitive scalp which may react slightly differently to the trauma of surgery, making them more prone to shock loss.

Hair Characteristics And Shock Loss: 

The type, density, and health of the existing hair can influence the likelihood and extent of shock loss. A healthy hair follicle that is not experiencing any miniaturization and is deemed permanent may be less prone to shock than a hair follicle that is shrinking and producing thinner hair.

Hair Health:

Individuals with pre-existing conditions like androgenic alopecia (pattern baldness) might experience different levels of shock loss compared to those without such conditions. As earlier mentioned, thinning miniaturized hair is at greater risk of shock.

Medication:

Certain medications prescribed for hair loss (e.g., minoxidil or finasteride) may influence the rate and extent of shock loss. Medications such as XYON Health’s topical dutasteride can impact temporary shock loss. When these medications increase the health of the hair follicle making it stronger, it may be less prone to shock.

While not everyone will experience shock loss after a hair transplant, the majority will and all patients should be prepared for it.

Will my hair grow back after Hair Transplant shock loss?

If the shock loss is telogen effluvium or temporary, yes, it will grow back. If it is not telogen effluvium and more of the permanent variety, the hair will not grow back.

What exactly is “permanent” shock loss?

Permanent shock loss is what the name implies, the loss is permanent and hairs damaged by transection (cutting across the existing hair follicle) will not grow back.

While temporary shock loss is expected and quite common, any permanent shock loss of viable hair can also occur and should be avoided. While temporary shock loss could be described as a phenomenon associated with indirect trauma, permanent shock loss can be the result of a more direct trauma. When making recipient site incisions among existing hair, those incisions must be precise and well executed so as not to cut into the neighboring follicles thus preventing them from surviving the procedure. The field of vision must also me optimal and certain technology must be employed.

There are a few factors that will greatly reduce the chance of permanent shock loss.

 

Small custom sized blades.

To successfully make incisions between existing hair without causing damage, in general the smaller the blade the better. If the blade size is not custom sized to the follicle and is larger than needed, the larger blade working in tight quarters has a greater chance of causing damage. In this case the incisions will need to be placed farther away from the existing follicle to prevent damage which will result in a lower density transplant.

The angle of the recipient site incisions must be exact and parallel to the existing hair. If the angle is slightly off and the incision cuts across the adjacent existing hair follicle and transects it, the follicle will no longer produce a viable hair. What should have been temporary shock loss then becomes permanent.

 When hair in the recipient area is too long, it can create a couple issues. At a certain length, the hair will obstruct the field of vision and also make it more difficult to assess the exit angle of the hair shaft. To precisely make an incision parallel to the existing follicle, the exact exit angle of this native hair must be determined. Longer hair can distort the exit angle making it more difficult to detect. Therefore, a shave of the recipient area is necessary. Once the hair shaft is shaved to the necessary length, the angle of that hair shaft can be established and the correct incision can be made parallel to the adjacent follicle. This will eliminate any transection or damage to the follicle or “permanent” shock loss.

Especially in cases where high density is desired and necessary, the surgeon must have an extensive understanding of how to avoid permanent shock loss and the necessary tools and expertise to prevent it. The more density required, the higher the degree of difficulty when transplanting among existing hair. Experienced surgeons who routinely achieve high densities will take the necessary steps mentioned above to produce the best outcome. Transplanting among existing hair without a shave, without custom blades, without a high degree of skill etc. may be possible without causing damage but the density will likely not be acceptable to the patient. At some stage, the patient will want a better result with more density and these steps will ultimately need to be employed.

What Does Hair Transplant Shock Loss Look Like?

Here are a few examples of what shock loss in the donor area might look like, including examples of recovery times and what the recovery process could look like as the area returns to normal.

In this FUT case, within the first 2 months post-op, the scar is healing nicely but some temporary shock loss was apparent in the donor area above and slightly below the scar.

At 3-4 months post-op, those fallen hairs started to grow back and there is virtually no evidence of the previously shocked follicles.

In this FUT case, the worse of the temporary shock loss was confined mostly to an area of the donor in the back. The loss was substantially above and below the scar line at 3 months post-op just before it started to grow back.

At 5 months post-op, those shocked follicles are producing hair again. From this point forward, the area will continue to improve each month until full maturation.

Hair Transplant Shock Loss Conclusion

In most hair transplant procedures, some degree of shock loss is likely to occur. Understanding that shock loss is usually temporary and a normal part of the hair transplant process can help manage expectations and alleviate some of the anxiety. Be prepared for the possibility of your hair looking thinner before it gets better.
